Position Description:

Will provide health maintenance care, episodic acute and chronic disease management (e.g. family planning, communicable disease care), clinical preventive services, and disease prevention services. Performs a full range of complex duties within a framework of established procedures and protocols and provides services that require the use of experienced judgment and skills. Acts as a clinician as assigned, obtains health histories, performs physical examinations, collects and interprets laboratory specimens and makes diagnoses. Delegates basic nursing services to nurses, licensed practical nurses, and unlicensed assistive personnel. Works closely with the nurses and physicians to support the coordination of services and proper implementation of nursing care and treatment plans, describes services that are available and eligibility criteria as appropriate, participates in the development and implementation of plan(s) of care on the health risks identified during the nursing assessment and provides basic nursing care based on determined needs. Initiates appropriate teaching and demonstration of care for individual clients and families based on nursing assessment with an emphasis on the promotion, maintenance, and restoration of health and self-care. Follows up on patients as often as indicated. Refers patients to appropriate resources. Maintains complete and accurate records on each patient seen. Participates in training related to job responsibilities. Provides in-service training to department personnel and representatives from other agencies. Assists with internal medical nursing record audits. Assists in the administration and direction of public health nursing services. May be assigned to different clinic/work locations as needed.

Minimum Requirements:

Bachelor's degree; Master of Science in Nursing from a program accredited by a nationally recognized nursing accreditation organization. Three (3) years of experience as a nurse practitioner preferred. Skill in oral and written communication. Ability to establish and maintain effective working relationships. Bilingual (Spanish/English) preferred. Current Nurse Practitioner Certification from a nationally recognized nursing accreditation organization. Certificate of Fitness issued by the State of Tennessee. Licensed as an Advanced Practice Nurse by the State of Tennessee. Cardiopulmonary Resuscitation (CPR) Certification. Valid class “D” driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.
